Engineered for Surgical Precision: The Next-Generation Mini Fragment Locking Instrument Set

Modern upper and lower extremity trauma surgery demands low-profile fixation hardware paired with intuitive, tactile instrumentation. Traditional hospital inventories often suffer from fragmented tray bloat—requiring separate instrument cases for volar distal radius plating, metacarpal fixation, and forefoot arthrodesis. Our unified 2.4mm/2.7mm Locking Instrument Set eliminates redundant tray turnover by integrating every reduction, drilling, depth-measuring, and torque-controlled insertion tool into a single high-efficiency Mini Fragment Locking Instrument Set.

Advanced Engineering: 2.4mm / 2.7mm Variable Angle Locking System Capabilities

Comminuted intra-articular fractures frequently present complex fracture lines, subchondral bone voids, or pre-existing K-wires that obstruct fixed-axis screw trajectories. To empower surgeons with intraoperative flexibility, this set is fully equipped as a 2.4mm / 2.7mm Variable Angle Locking System. Its specialized dual-ended drill guides allow surgeons to choose between nominal fixed-angle coaxial locking or polyaxial angulation depending on anatomical necessity:

  • Fixed-Angle Coaxial Thread Engagement: Threaded LCP drill sleeves thread directly into standard combi-holes and fixed locking holes for dead-center coaxial trajectory when rigid fixed-angle construct stability is required.
  • Calibrated Torque Limiters: Audible and tactile click-release torque limiters prevent over-torquing in variable-angle holes, preserving the ability to revise or redirect screws up to three times without compromising lock strength.

Multi-Specialty Clinical Indications Across Upper & Lower Extremities

By bridging 2.4mm and 2.7mm screw instrumentation in one consolidated platform, hospitals and surgical centers maximize capital ROI across Orthopedic Trauma, Hand & Microvascular Surgery, and Podiatric Foot & Ankle services.

Upper Extremity Trauma: Distal Radius Locking Plate Instrument Set & Hand and Wrist Locking Instrument System

In upper extremity trauma, articular surface restoration and tendon preservation depend on ultra-precise screw placement near the watershed line and subchondral bone plate. Configured as a dedicated Distal Radius Locking Plate Instrument Set, the tray provides specialized radiolucent retractors, pointed reduction forceps with speed-locks, and low-profile variable-angle drill guides tailored for volar two-column, dorsal, radial styloid, and fragment-specific distal radius plating.

Simultaneously, the set functions as a comprehensive Hand and Wrist Locking Instrument System for delicate carpal, metacarpal, and phalangeal osteosynthesis. Fine-tipped periosteal elevators, 60mm ultra-sensitive depth gauges, and in-situ plate benders allow hand surgeons to contour T-plates, Y-plates, condylar plates, and wrist fusion plates without damaging locking hole threads.

  • Distal Radius & Ulna Fractures: Extra-articular (AO 23-A), partial articular (AO 23-B), and complex intra-articular (AO 23-C) volar, dorsal, and radial column fixation; ulnar shortening osteotomies.
  • Carpal & Wrist Procedures: Total wrist arthrodesis, four-corner arthrodesis, intercarpal fusions, and complex perilunate fracture-dislocations.
  • Metacarpal & Phalangeal Reconstruction: 2.4mm shaft, neck, and periarticular base fractures (including Bennett and Rolando fracture patterns) and corrective rotational osteotomies.

Lower Extremity Reconstruction: Foot and Ankle Locking Compression Instrument Set

For lower extremity trauma and elective deformity correction, the system transitions effortlessly into a heavy-duty Foot and Ankle Locking Compression Instrument Set. The inclusion of robust 2.7mm cortical compression drill guides, olive temporary fixation wires, and bone-holding forceps allows foot and ankle specialists to achieve high interfragmentary compression across midfoot and hindfoot joints while maintaining rigid locking construct stability.

  • Forefoot & Midfoot Fractures/Fusions: First metatarsophalangeal (MTP) joint arthrodesis, Lapidus (1st TMT) arthrodesis, Lisfranc fracture-dislocation stabilization, and 2nd–5th metatarsal shaft fractures.
  • Navicular, Cuboid & Cuneiform Fixation: Bridge plating and variable-angle locking stabilization of high-energy midfoot crush injuries and Charcot neuroarthropathy reconstruction.
  • Distal Fibula & Calcaneal Fractures: 2.7mm lateral/posterolateral distal fibula locking plate instrumentation and mini-fragment calcaneal perimeter plating.

Sterile Processing (SPD) Efficiency: Modular Orthopedic Small Bone Trauma Instrument Tray

Hospital Sterile Processing Departments (SPD) and operating room managers evaluate surgical systems on sterilization throughput, weight compliance, and visual inventory verification. Our Orthopedic Small Bone Trauma Instrument Tray is housed in a perforated, hard-anodized aluminum sterilization case with non-degrading medical silicone brackets.

CLINICAL SAFETY & OPERATIONAL PROTOCOL: 2.4MM/2.7MM LOCKING INSTRUMENT SET

Perioperative Safety, Handling, Cleaning & Sterilization Guide for the 2.4mm/2.7mm Locking Instrument Set, Distal Radius Locking Plate Instrument Set and Hand and Wrist Locking Instrument System

SECTION 01 | PURPOSE, SCOPE & TRAINING REQUIREMENTS

1.1 Why This Protocol Exists

This Clinical Safety & Operational Protocol establishes mandatory steps for checking, preparing, using, cleaning, and sterilizing the 2.4mm/2.7mm Locking Instrument Set and its matching trauma implants. Following these rules prevents accidental bone fractures, heat damage to bone cells, retained metal fragments, stripped screws, galvanic corrosion, and surgical infections.

1.2 Scope: Distal Radius Locking Plate Instrument Set & Hand and Wrist Locking Instrument System

This protocol applies to all reusable manual, modular, hollow (cannulated), impact, and power-assisted tools in the 2.4mm/2.7mm Locking Instrument Set, including the Distal Radius Locking Plate Instrument Set and the Hand and Wrist Locking Instrument System:

  • Cutting & Bone Preparation Tools: Drills, taps, osteotomes, chisels, gouges, rasps, and bone awls.
  • Hollow (Cannulated) & Guided Tools: Cannulated drills, guide wires (K-wires and Steinmann pins), depth gauges, trocars, and radiolucent carbon-fiber targeting frames.
  • Torque & Ratcheting Mechanisms: Calibrated torque-limiting drivers, ratcheting handles, and screw reduction towers.
  • Osteosynthesis & Trauma Fixation Implants: Anatomical locking and non-locking bone plates, plus cortical, cancellous, and cannulated screws supplied in sterile packs or sterilization caddies.

SECTION 02 | CRITICAL SAFETY WARNINGS & MATERIAL RULES

CRITICAL WARNINGS FOR SMALL-BONE SURGERY

  • Never use dull drills or taps: Dull cutting flutes require extra pushing force and heat cortical bone above 47°C (116.6°F), which kills bone cells (thermal osteonecrosis) and causes screws to loosen later
  • Never pry or bend fine tools: Hollow drills, taps, osteotomes, and guide wires in the 2.4mm/2.7mm Locking Instrument Set are designed strictly for straight-ahead (coaxial) loads. Off-axis prying causes sudden brittle fracture and leaves metal pieces inside the bone.
  • Never mix brands or incompatible metals: Never mix tools or implants from different manufacturers. Never combine Stainless Steel screws with Titanium or other material plates, which triggers battery-like galvanic corrosion.
  • Strict single-use rule: Never reprocess single-use items like K-wires or disposable blades marked “Do Not Reuse”. Any screw or plate that touches blood, bone, or tissue is strictly single-use and must never go back into the implant caddy.

2.1 Contraindications & Material Precautions

  • When Not to Use: Do not use any instrument from the Distal Radius Locking Plate Instrument Set showing rust, pitting, cracks, bent shafts, mushroomed strike plates, faded markings, or expired torque calibration. Do not implant hardware in patients with active infection, severe metal allergy, insufficient bone stock, or open pediatric growth plates.
  • Alloy & Polymer Care: Dry hardened stainless steel immediately after washing to avoid chloride pitting. Keep color-anodized titanium away from bleach, peroxide, and alkaline cleaners above pH 9.5, and never steam-sterilize PEEK, Radel PPSU, or Ultem plastics above 137°C

SECTION 03 | PRE-OPERATIVE INSPECTION & SAFE OR HANDLING

3.1 Inspection & Torque Verification

Inspect every tool in the Hand and Wrist Locking Instrument System and the 2.4mm/2.7mm Locking Instrument Set on the OR back table before incision:

  • Instrument & Implant Checks: Check cutting flutes for chips or heat discoloration, pass a wire stylet through hollow drills and depth gauges to confirm zero rust or blockage, and test forceps, ratchets, and quick-connects for smooth locking. Confirm primary, smaller (-1), and larger (+1) implants 24 hours before surgery.
  • Torque-Limiting Drivers: Recalibrate reusable torque drivers every 12 months or 100 sterilization cycles (within ±10% tolerance). Stop turning immediately after the first audible “click” to prevent clutch damage or screw stripping.

3.2 Intraoperative Safety, Breakage & Point-of-Use Cleaning

  • Drilling, Contouring & No-Touch Rules: Thread drill sleeves fully into the plate, hold drills straight to avoid shaving metal slivers, and irrigate continuously with cooled sterile 0.9% saline using “peck drilling” every 3–5 seconds to keep bone below 47°C. Bend plates only across inter-hole bridges with system bending irons—never reverse-bend or deform threaded holes. Change outer gloves before touching implants, never scratch implants with toothed clamps (which reduces fatigue strength by over 50%), and place any caddy screw that touches blood or bone into a “Discard Basin”.
  • Broken Tool Steps: If a drill or tap breaks, stop immediately, fit pieces together on the back table, take AP and Lateral X-rays, weigh extraction risks, record any retained fragment for MRI safety, and quarantine the broken device.
  • OR Wipe-Down (Within 15–30 Minutes): Wipe blood and bone off tools right after use with sterile sponges soaked only in sterile water (never use 0.9% saline, which causes rapid rust pitting). Flush hollow bores with sterile water and apply pH-neutral enzymatic foam before transport.

SECTION 04 | CLEANING, STERILIZATION & STORAGE

  • CSSD Washing: Disassemble multi-part tools from the Mini Fragment Locking Instrument Set completely, soak 10–15 minutes in warm (30°C–40°C) pH-neutral enzymatic detergent (pH 6.0–8.5), and scrub underwater with soft nylon brushes. Brush hollow tubes 3 times, flush at 30 PSI for 30 seconds, ultrasonic clean at 35–45 kHz for 10 minutes, and run an ISO 15883 washer-disinfector cycle (cold pre-wash <35°C, main wash 45°C–55°C, RO/DI rinse, 90°C–93°C disinfection, and HEPA drying at 90°C–110°C). Wash implant caddies in separate implant-only loads.
  • Autoclave & Storage: Lubricate joints only with water-soluble “instrument milk,” place two Type 5 chemical indicators per tray, and keep tray weight at or below 25 lbs (11.3 kg). Sterilize using Pre-Vacuum steam at 132°C (270°F) for 4 minutes (US) or 134°C–137°C (273°F–279°F) for 3 minutes (EU) with 30–45 minutes drying; gravity and flash (IUSS) cycles are prohibited. Cool on wire racks for 60–120 minutes, reject any wet pack, and store at 18°C–24°C and 30%–60% humidity.
